氟比洛芬酯复合布托啡诺用于开胸手术镇痛的临床观察

摘 要：
目的:观察氟比洛芬酯复合布托啡诺用于开胸手术患者镇痛效果和不良反应。方法:ASAⅠ或Ⅱ级择期行开胸手术患者60例,随机分为A、B、C组。A组于关胸前给予氟比洛芬酯50 mg和阿扎司琼10 mg,术后PCIA镇痛给予氟比洛芬酯200 mg+阿扎司琼10 mg+0.9%氯化钠至100 ml。B组于关胸前给予布托啡诺1 mg和阿扎司琼10 mg,术后PCIA镇痛给予布托啡诺10 mg+阿扎司琼10 mg+0.9%氯化钠至100 ml。C组于关胸前给予氟比洛芬酯50 mg和阿扎司琼10 mg,术后PCIA镇痛给予氟比洛芬酯150 mg+布托啡诺5 mg+阿扎司琼10 mg+0.9%氯化钠至100 ml。PCIA泵设置,维持量2 ml/h,单次负荷剂量0.5 ml,锁定时间15 min。观察术后1、6、12、24、36、48疼痛、镇静评分及不良反应发生率。结果:术后6～24h各时点,A组VAS评分高于B、C组(P<0.05);A、C组术后1～24h各时点Ramsay镇静评分低于B组(P<0.05);A、C组头晕、嗜睡发生率明显低于B组(P<0.05);各组均无呼吸抑制发生。结论:氟比洛芬酯复合布托啡诺用于开胸手术,镇痛效果确切,不良反应少,是一种安全可行的镇痛方法。

摘 要：
Objective:To observe the analgesic effect and adverse reaction of flurbiprofen axetil with butorphanol in thoracotomy.Methods:60 patients with ASA I～II stage for selective thoracotomy were divided randomly into group A,B and C.Those in group A were treated with flurbiprofen axetil 50mg and azasetron 10mg before chest closure,along with flurbiprofen axetil 200mg and azasetron 10mg in 100ml postoperative PCIA paregoric pump;those in group B were treated with butorphanol 1mg and azasetron 10mg before chest closure,along with butorphanol 10mg and azasetron 10mg in 100ml postoperative PCIA paregoric pump.Those in group C were treated with flurbiprofen axetil 50mg and azasetron 10mg before chest closure,with flurbiprofen axetil 200mg,butorphanol 5mg and azasetron 10mg in 100ml postoperative PCIA paregoric pump.Parameters of PCIA paregoric pump were set by 2ml/h for maintenance dose,0.5ml for single loading dose and 15min for locking time.The visual analogue scale,ramsay sedation scores and adverse reaction incidence were recorded at time point of 1,6,12,24,36,48h after operation.Results:Between 6～24h,VAS of group A was higher than that of group B and C(P<0.05).At the point of 1～24h,Ramsay of group A and C was lower than that of group B(P<0.05).The incidence of postoperative dizziness and drowsiness in group A and C was lower than that in group B(P<0.05).There was no respiratory depression in neither of group.Conclusion:flurbiprofen axetil combined with butorphanol is an effective and safe analgesic regime for thoracotomy,which has little adverse reaction incidence.
