地佐辛复合利多卡因和罗哌卡因改良颈丛阻滞用于甲状腺手术的临床研究

摘  要：
目的:观察地佐辛复合低浓度利多卡因罗哌卡因行颈丛阻滞用于甲状腺手术的麻醉效果和安全性。方法:选择择期行单侧甲状腺次全切除术患者100例,ASAⅠ~Ⅱ级,随机分为A组(地佐辛0.1mg/kg复合0.67%利多卡因+0.25%罗哌卡因)和B组(0.67%利多卡因+0.25%罗哌卡因),每组各50例,行改良颈丛阻滞,患侧行C3、C4深丛阻滞,各注药5ml,双侧颈浅丛阻滞,各注药10ml。观察两组的麻醉起效时间、感觉阻滞持续时间、阻滞效果及心血管不良反应。结果:A组麻醉起效时间明显短于B组,感觉阻滞持续时间明显长于B组,麻醉满意度优于B组,差异均有统计学意义(P<0.05),B组的SBP、DBP、HR较A组明显升高,差异有统计学意义(P<0.05)。结论:地佐辛复合利多卡因和罗哌卡因行改良颈丛阻滞用于甲状腺手术具有麻醉效果好,牵拉反应少,镇痛持续时间长,心血管不良反应少等优点。

摘  要：
Objective:To observe the effects of dezocine combined low concentration lidocaine ropivacaine for cervical plexus block anesthesia effect and safety of thyroid operation.Methods:patients undergoing unilateral subtotal thyroidectomy in 100 cases,ASA Ⅰ-Ⅱgrade,were randomly divided into group A(dezocine 0.1mg/kg compound lidocaine0.67%+0.25%ropivacaine)and group B(0.25%ropivacaine+0.67%lidocaine ropivacaine)in all 50 cases,the improvement of cervical plexus block,the ipsilateral C3,C4 deep plexus block,the injection of 5ml,bilateral superficial cervical plexus block,the injection of 10 ml.To observe the onset time of anesthesia in group two,duration of sensory block,block effects and adverse cardiovascular response.Results:in group A,anesthesia onset time was faster than that of B group,A group,duration of sensory block was significantly longer than those of group B,group A anesthesia satisfaction than that of B group,the difference was statistically significant(P<0.05),group B,SBP,DBP,HR were significantly increased in group A,the difference was statistically significant(P<0.05).Conclusion:Dezocine combined lidocaineand ropivacaine in cervical plexus block anesthesia for improved has good effect of thyroid operation,less analgesic traction reaction,long duration,cardiovascular advantages of fewer adverse reactions,is safe and effective.
