营养支持联合免疫调节治疗老年慢性阻塞性肺疾病急性发作期的临床效果

摘  要：
目的:观察老年慢性阻塞性肺疾病(COPD)急性发作期行营养支持联合免疫调节治疗的临床效果。方法:选取济宁市第一人民医院2016年1月—2018年1月收治的COPD急性发作期患者200例,随机分为观察组108例和对照组92例,对照组给予营养支持治疗,观察组在对照组治疗基础上行免疫支持治疗,观察两组治疗后的临床效果。结果:治疗后,观察组临床治疗总有效率明显高于对照组(χ~2=8. 61,P <0. 01),外周血管淋巴细胞(TLC)、Ig A以及T淋巴细胞(CD4~+、CD8~+、CD4~+/CD8~+)等免疫指标明显优于对照组(P <0. 05或P <0. 01),体重、TP、ALB等营养指标和Pa O2、SaO_2、PaCO_2等血气分析指标也明显优于对照组(P <0. 05或P <0. 01)。结论:对老年COPD急性发作期患者在营养支持基础上给予免疫调节治疗能明显改善患者营养水平和免疫功能,增加肺通气功能,提高临床治疗效果。

摘  要：
【Objective】 To observe the clinical effect of nutritional support combined with immunomodulation in the acute stage of senile chronic obstructive pulmonary disease( COPD). 【Method】 Totally 200 patients with acute onset COPD admitted to our hospital from January 2016 to January 2018 were randomLy divided into the observation group( 108 cases) and the control group( 92 cases). The control group was treated with nutritional support. The observation group was treated with basic uplink immunotherapy,and the clinical effects of the two groups were observed. 【Result】 After treatment,the clinical total effective in observation group was obviously higher than that of control group( χ~2= 8. 61,P < 0. 01),immune indexes including peripheral vascular lymphocyte( TLC),IgA and T lymphocytes( CD4~+,CD8~+,CD4~+/CD8~+) were obviously better than that of control group( P < 0. 05 or P < 0. 01),nutrition indicators including body weight,TP and ALB and blood gas analysis index including PaO_2,SaO_2 and PaCO_2 were obviously better than that of control group( P <0. 05 or P < 0. 01). 【Conclusion】 Immunomodulation therapy based on nutritional support for elderly patients with acute onset of COPD can significantly improve the nutritional level and immune function of patients,increase pulmonary ventilation function and improve clinical treatment effect.
