临床营养师在营养支持小组中的作用及临床实践

摘 要:
目的:临床营养师在营养支持小组(nutrition support team,NST)中具有重要作用,但在很多儿童专科医院中并未受到重视。本文分析2011—2013年上海儿童医学中心临床营养师开展营养支持治疗工作的情况,旨在为我国儿科营养学的发展提供依据。方法:回顾性分析上海儿童医学中心临床营养科2011年1月—2013年12月期间临床营养师会诊工作情况,总计患儿414例。用统计学分析方法分析会诊原因、随访率、会诊科室类别、随诊率、营养指导方式、患儿会诊前后营养状况比较等。结果:2011—2013年上海儿童医学中心临床营养科会诊量以22%—38%的速度逐年增加,申请会诊科室由原先的10个增加到15个。营养不良患儿体重由干预前的13.22kg提高到13.41kg(P<0.01)。营养风险评分由干预前的STAMP评分=5分下降至干预后的4.23分(P<0.01)。在会诊中,喂养指导人数占69%、疾病饮食指导人数占31%。结论:在会诊治疗过程中,临床营养师应与临床医生、护士紧密联系,开展营养评估与营养治疗,及时筛查出有营养风险的患儿并进行针对性的营养干预和营养宣教,最终改善患儿营养状况及临床结局。

摘 要:
Objective Clinical nutritionist plays an important role in the nutrition support team,but has not been paid attention by Children's Hospital. We analyzed the data about the works of clinical dietitians in Shanghai Children's Medical Center( SCMC) in 2011—2013 to afford references for pediatric nutrition. Method We retrospectively explored 414 pediatric patients who got clinical dieteticans consultation during January 2011-December 2013,and analyzed the reasons of consultation,follow-up consultation rate,department category,the mean of nutritional advise,and the nutritional status before and after the consultation. Result The total number of consultation increased by 22% —38%,and the involved departments increased from 10 to 15. Malnourished pediatric patients gained body weight from13. 22 kg to 13. 41 kg after dietitian consultation( P < 0. 01). We used STAMP score to evaluate the nutritional risk of inpatients. The mean score was 5 before our consultation and 4. 23 after that( P < 0. 01). The proportion of feeding instruction and disease diet instruction was 69% and 31%,respectively. Conclusion Clinical dietitians should work closely with clinicians and nurses and assess nutrition status of inpatients to find out patients with nutritional risk,and give them tailored nutrition intervention and nutrition education to improve the nutritional status and clinical outcome.
