338例胃癌患者营养风险筛查及营养治疗的效果

摘  要:
目的:采用营养风险筛查2002 (NRS 2002)评估338例胃癌患者营养风险并探讨营养治疗对胃癌患者的效果.方法:于2015年1月-2017年6月选取宜宾市第一人民医院住院治疗的胃癌患者338例,采用NRS 2002评估其营养风险,并将存在营养风险的202例患者随机分为治疗组(107例)和对照组(95例),分别给予个性化精准营养支持和家属自备饮食,观察并比较其一般住院指标、营养状况指标和远期生存质量等.结果:338例胃癌患者有202例存在营养风险,发生率59. 8%;治疗组住院时间、住院费用、并发症发生率和1年再入院率均低于对照组(P<0. 05);治疗组治疗后营养相关指标(体重、白蛋白、前白蛋白和血红蛋白)优于治疗前(P<0. 05),治疗组营养相关指标治疗前后差值优于对照组(P<0. 05);癌症患者生活质量评价量表QLQ-C30 (躯体、情绪、恶心呕吐和食欲下降)和QLQ-STO22 (疼痛、反流、饮食受限、焦虑和味觉改变)评分治疗组高于对照组(P<0. 05).结论:胃癌患者营养风险发生率高,个体化精准营养支持不仅能缩短住院时间、降低术后并发症、改善患者营养状况甚至提高患者远期生存质量,针对胃癌患者制定个体化营养支持方案,有利于提高疾病预后和患者远期生存质量.

摘  要:
【Objective】 To screen nutritional risk of 338 gastric cancer patients by Nutritional Risk Screening 2002( NRS 2002) and assess the effects of nutritional therapy. 【Method】 From January 2015 to June 2017,338 gastric cancer inpatients were selected from the first peoples' hospital of Yibin to screen nutritional risk by NRS 2002,202 patients with nutritional risk were divided into therapy groups( n =107) and control group( n = 95),and respectively given with precise nutritional support and family-prepared homogenate nutrient solution,finally the general hospitalization index,nutritional status and long-term life quality of patients were observed. 【Result】 The incidence rate of nutritional risk was 59. 8% among gastric cancer patients. The hospitalization duration,hospitalization expense,incidence rate of complications and postoperative radiotherapy and chemotherapy rate of therapy group were significantly lower than control group( P<0. 05). The nutrition related indexes of after nutritional therapy were better than those of before therapy( P<0. 05),the difference before and after therapy in therapy group was significantly superior than control group( P<0. 05). The some dimensions of long-term life quality scale( function of body and emotions,nausea and vomiting,loss of appetite in QLQ-C30,pain,reflux,diet restriction,dysphoria and dysgeusia in QLQSTO22) in therapy group was higher than control group( P<0. 05). 【Conclusion】 The incidence rate of nutritional risk is high among gastric cancer patients. Positive precise nutritional support could shorten hospitalized duration,reduce postoperative complications,improve nutritional status and long-term life quality of the patients,so it is essential to develop an individualized precise nutritional support scheme for gastric cancer patients to improve prognosis and long-term quality of life.
